ATF Paper Forms Library

All ATF forms can be downloaded in PDF format to be filled out, printed, and physically mailed in for processing. There are a few advantages to submitting your forms the old-fashioned way, including receiving a physical tax stamp when your application is approved.

• ATF Forms 1, 2, 3, 4, 5, 6, 6A, 7, 8, 9, 10 and 5300.11 are available for PDF download (links below).

Note:  Paper is currently the only option available for Form 4

• Form 1 (5320.1) – Application to Make and Register a Firearm
  • Filed by an individual, entity, or trust to get approval to make and register an NFA firearm.
• Form 2 (5320.2) – Notice of Firearms Manufactured or Imported
  • Filed by a qualified manufacturer or importer to register NFA firearms manufactured or imported.
• Form 3 (5320.3) – Application for Tax-Exempt Transfer of Firearms and Registration to Special (Occupational) Taxpayer (National Firearms Act)
  • Filed by a qualified Federal firearms licensee to transfer to another qualified federal firearms licensee.
• Form 4 (5320.4) – Application for Tax Paid Transfer and Registration of Firearm
  • Filed by an individual, entity, or trust to transfer and register an existing NFA firearm.
• Form 5 (5320.5) – Application for Tax-Exempt Transfer of Firearms and Registration of Firearm
  • Filed by a current registrant when firearm is being transferred to or from a Government entity.
• Form 9 (5320.9) – Application and Permit for Permanent Exportation of Firearms
  • Filed by a qualified Federal firearms licensee for the exportation of NFA firearms
• Form 10 (5320.10) – Application for Registration of Firearms Acquired by Certain Governmental Entities
  • Filed by State or local government agencies to register unregistered NFA firearms they acquire and want to use for official business
• Form 5320.23 – National Firearms Act (NFA) Responsible Person Questionnaire
  • When filing using a TRUST, must be included along with fingerprints on Form FD-258 and photograph, when submitting a Form 1, Form 4, or Form 5 to the ATF. Additional copy must be directed to the responsible person’s chief law enforcement officer. READ ALL INSTRUCTIONS!

• Form 6 Part 1 (5330.3A) – Application and Permit for Importation
  • Filed to request permission to import a firearm, ammunition or implement of war.
• Form 6 Part 2 (5330.3B) – This form is used to apply to permanently import firearms, ammunition and implements of war. FOR USE BY ACTIVE DUTY MILITARY MEMBERS ONLY.
  • Filed to request permission to import a firearm, ammunition or implement of war.
• Form 6A (5330.3C) – Release and Receipt of Imported Firearms, Ammunition and Implements of War
  • Filed by a Federally licensed importer subsequent to an approved Form 6 permit and presentation to CBP for the release and verification of the release of the items from Customs custody.

• Form 7 (5310.12 / 5310.16) – Application for Federal Firearms License
  • Filed to request permission to import a firearm, ammunition or implement of war.
• Form 8 (5310.11) Part II – Federal Firearms License (FFL) Renewal Application
  • Filed by an existing FFL and typically sent to the FFL address on file automatically 90 days prior to FFL expiration.

Annual Firearms Manufacturing and Exportation Report (PDF Download)

• Form 5300.11 – Annual Firearms Manufacturing and Exportation Report Under 18 U.S.C Chapter 44, Firearms
  • This form is required of all Type 07 and Type 10 federal firearms licensees (FFLs) for reporting production data for the manufacture and exportation of firearms.
